As the year comes to a close, it’s common to look back and realize that your annual goals are still far from being achieved. That sense of falling behind can be overwhelming, and it’s easy to feel like time is slipping away faster than you expected.
To combat this, the 12 Week Year method, introduced by Brian P. Moran and Michael Lennington, shifts away from traditional 12-month planning in favor of a 12-week cycle. This approach encourages rapid goal achievement through focused execution, helping you get more done in less time.
Building on the principles from the book, we’ve created a 12 Week Year Plan Visualization Notion Template to help you turn your big yearly goals into clear, actionable plans. The key idea is to accomplish more in a shorter time by staying focused and organized. With this template, you’ll be able to set clear intentions, break down your goals into manageable steps, and stay on track every week. The results might surprise you—what you can achieve in 12 weeks can far exceed what most people accomplish in a whole year.
The template is designed with several key features to make goal-setting clearer, more visual, and easier to follow:
Clear and logical structure for goal setting and execution:The template includes three interconnected databases—Goals, Weekly Reviews, and Daily Tasks. This clear structure helps you break down your goals in an organized way, making execution systematic and achievable.
Task visualization at a glance:With dashboards that display goal countdowns, progress tracking, weekly plans, and overdue tasks, the template brings the 12 Week Year’s focus on visualized planning to life. This makes it easy to plan and execute tasks effectively, ensuring you stay on top of everything.
Convenient automation for higher efficiency:Automation features like quick buttons to generate weekly tasks or create review pages for each 12-week cycle reduce manual work, allowing you to focus more on achieving your goals and less on administrative tasks.
With this Notion template, you'll not only gain clarity but also the tools to follow through on your plans efficiently, ensuring you make real progress in just 12 weeks.

Goal Page
The goal page leverages Notion's new custom layout feature to organize key information, data properties, task tracking, and summary reports into separate sections. Here's an overview of the layout:
Basic Information Module: Includes essential details like the goal’s time range, time progress, and task progress.
Property Setting Module (Fill in goal properties here): Users only need to set three key attributes: Goal Status, Start Time, and Goal Category.
Weekly Summary Module: Archives weekly summaries, including task completion rates and scores, helping users reflect on their progress.
Current Week Progress Dashboard Module: Tracks the current week’s time progress, task progress, and overdue tasks.
In addition to these properties, the goal page includes a section for textual descriptions inspired by the 12 Week Year. Using leading indicators and lagging indicators, users can define their goals and track progress more effectively:
Overall Goal: Clear and measurableGoals should specify both the desired result and how to achieve it. For example, instead of simply writing “Lose 5kg,” include “by exercising five times a week and following a healthy diet.”
Leading IndicatorsTrack actionable behaviors that influence goal achievement, such as workout frequency, specific exercises, calorie intake, or water consumption. These indicators focus on process-oriented actions for real-time evaluation.
Lagging IndicatorsMeasure the end results of your efforts, such as losing 1kg every 4 weeks. These indicators help adjust your pace and maintain motivation by reflecting actual outcomes.
To streamline planning, the template offers a one-click button to generate 12 weekly review pages and automatically link them to the weekly plan database. It’s recommended to define weekly objectives first to make planning straightforward.
Weekly Review Page
The weekly plan page helps users track and visualize progress for each week. Its database attributes include:
Basic Information Module: Displays the goal’s time range, time progress, and task progress.
Property Setting Module (Fill in weekly plan properties here): Users need only provide four key attributes: Weekly Status, Strategies, Linked Goal, and Start Time—simple and efficient.
Current Week Progress Dashboard Module: Tracks weekly time progress, task progress, and overdue tasks for better monitoring.
Additional features of the weekly review page:
Detailed Leading and Lagging IndicatorsWeekly indicators should be more specific. For instance, leading indicators could include workout duration, frequency, and exact calorie intake, while lagging indicators track weight loss in kilograms.
Daily Task TrackingTasks added to the daily task database are automatically linked here. Completion updates the progress dashboard in real time.
Weekly SummaryOn the last day of the week (Sunday is recommended), reflect on your execution using Execution Measure and Results Measure. Execution Measure evaluates progress based on leading indicators and daily tasks, while Results Measure assesses outcomes using lagging indicators. Assign a score (0–100) for the week based on execution, and set the page status to “Reviewed” once completed.
Evaluation and Adjustment
If your weekly score falls below 85%, identify the root cause. Was it an overloaded schedule, poor planning, or lack of discipline?
Adjust next week’s plan and priorities accordingly to improve completion rates.
This template eliminates the overwhelm of long-term annual planning by focusing on a compact 12-week cycle. The clear, visualized design ensures you stay on track at every stage while maintaining motivation. Start your 12-week high-efficiency goal plan today!
